Advance Care Planning
The Rebecca Sudore quote says it all when it comes to advance care planning: "The forms are only as good as the conversations that go with them.”
Many of us think of advance care planning (if we think of it at all) as a form to be completed and then you’re done! But the truth is much more nuanced. Someone may complete a form without telling anyone else what’s in it, and what a shock to find out you are someone’s healthcare proxy! The form may be un-findable, shoved in an unmarked box, or maybe it was completed years ago and opinions have drastically changed.
For all these reasons, we encourage you to TALK about your wishes with multiple parties: your healthcare proxy, the people who aren’t your healthcare proxy, but who may have a stake in your care like friends and family members, and your doctors. In this way you’re covered in multiple ways and there’s less to debate when tensions and emotions are high.
