Advance care planning documents
We had a great conversation on Advance Care Planning with Mettle Health founder BJ Miller last Friday. Many of the questions were related to the different types of documents like Advance Directives, POLSTS and DNR forms so we wanted to provide this quick hit on the basics of each form. Most important thing to note: advance care planning documents are inherently confusing. It's a very clunky process to project yourself into some future state that you can't quite know now, and to project what you wish for your care at that future date. There is no magical, singular document that does everything so it's important to keep your advance care planning documents updated as well as communicate your wishes with your family, friends and doctors so they understand WHY you have made the choices you made about your future care.
